I tried this and it seemed to work, for the most part. What I did was create a new country, something like US 2, and made African the only nationality. Then I created my fictional Negro League. I believe I had to delete all the players the game created for the league, then create new fictional players to get only Black players.
A few mistakes I made was making sure Negro League teams couldn't trade with other major leagues and that Negro League teams couldn't sign players from other leagues. I also didn't block players from independent leagues. And, at first, I assigned countries like Cuba to the Negro League. These mistakes meant that every once in a while a non-Black player appeared in the Negro League. (Apparently there are some Caucasian players in the game's default Cuba.)
These occasional non-Black players weren't a big deal. I just changed their nationality to African. But you don't want too many or else it becomes tedious.
Anyway, what you want to do, in my opinion, is possible by creating a new country, giving it one nationality (or maybe more if there are other nationalities that are only Black), and setting your league to that country. You might also want to understand the different nationalities OOTP provides. I don't, which is why I didn't realize some Cuban players are listed as Caucasian.
