I don't think that it was intentionally removed, but I think the 58 Sandy Alcantara All-Star is no longer in packs.
I assume it was originally possible to get it from the blank card error. But when that was fixed, and the Unsung Hero irons started appearing more frequently in packs Sandy was mistakenly left behind.
EDIT: There were also two Live Curtis Granderson cards (I think they had slightly different ratings), I'm not sure if/how that was resolved.
