Fear of salary initiates nasty chain reaction. That's the main trouble for me ATM. Algorithm is always the same:
AI can't extend player->he goes to trading block
Usually it's ok, they won't gift him to you. But most of these players are high earning vets, so salary fear turns into salary dumping and AI not only will gift him to you but also gift-wrap him with good prospect too. I described it in separate thread, but it's basically:
1) 5 star guy on playoff team with good chances for postseason, but makes 5+mil - "please take him, good deal, ok i agree"
2) 4 star guy but small salary - "worst deal I have ever seen" for same package

As for OP points: the only problem is that AIs are quite passive in Rule 5. Especially with possibility of 40-man roster shrinking (I had rule change to 38). Rebuilding teams with weak positions doesn't take "ready to start" prospects with potential? Meanwhile you can take worst team and rebuild from ground up in 1-2 years.
Most other things and self-limitations have to do with timing or in other words part of season. Rule 5 might be hard to fix because FA is still going. I usually have players that get "Minor with Major option" offer still thinking it through by the time Rule 5 happens. So roster slots+free CAP+uncertainty about FA are very tough to balance and code. My suggestion would be to have Rule 5 between FA players released from previous team's roster into pool and before FA actually starts. 1 day event and then FA. This would unclog this situation and produce less waivers, that are only for user to be taken because everybody else is at maxed roster spots.
