Both "features" you mention have been in OOTP for a long time.
1) If you are playing in one-pitch mode, the only time you see a pickoff throw by the AI is when the runner is actually out. The pitcher may actually throw over many more times than what you see, but if a throw does not result in an out, they are considered are non-plays they do not interrupt the game (like foul balls). Four pickoffs in one game is a lot, but not outside of realistic boundaries.
And for #2, as mars001 mentions, it happens fairly frequently. You can always replay the PbP and see what occurred if you want.
