Never really experienced this problem but I never tried to recreate it either. I think part of the problem could have been reserve rosters as reserve rosters tend to not follow the rules since it's not active. Because the setting is "foreign players allowed on active roster" so the reserve roster doesn't count towards the rule. That is loophole is probably what needs to be closed.

Or I know say the Japanese league by default has a foreign player limit but its minor league does not. So it gets a lot of foreigners. Now I'm not sure if that is IRL correct or not.

The only default league that I know foreign are banned in is Cuba but they don't allow outside free agents anyway.

I don't do any fictional leagues and just stick with all of the built in leagues. So I've only noticed in Japan they sign foreigners and put them in the minors.

Oh I also don't think the "spring training" roster counts as well because it usually calls up the team's entire reserve roster so I've seen Japan teams have >4 foreign players during spring training. Then they usually get bumped to reserve or minors.
