As for your theory, jax, that might explain why the throw over ALWAYS results in a pickoff - if it only shows the successful ones, that makes sense. But even so, why would the computer be successful so frequently with 'C" rated 'hold runners' pitchers against A/A, A/B, and B/A rated players? It makes no sense from that perspective.
Like I said, if it was only or even mostly happening to guys with D or E ratings for stealing, I wouldn't even bat an eyelash. The fact that it happens with such frequency to my best runners is what infuriates me.
FWIW, It mostly seems to happen in situations where the computer has scored in the previous inning and there are one or two outs with the man aboard - that could just be me noticing those more frequently because they irritate me more, though.
But even that aside, if I were getting one guy picked off every 7 games, I could probably live with that. 67%+ of the games played, no.

Well for what its worth I'm in my 5th season of playing out every game myself with ootp5 including about 10 games with the new patch and haven't seen this either. I can't believe this is an across the board issue or you would have had 50 responses in the first day saying "yeah this is happening to me too" so it sounds like something unique to your league.
Is the 67% figure you are using based on perception or are you keeping track? If it's just based on perception you should consider writing down each time this happens and the situation it happens in along with any other league information so that someone else can test this in the same situation and see if they come up with the same percentages you are getting.
Does it happen in other leagues or is this the only league you have going? If it's the only league you are playing I'd set up another league and test it that way. If it still happens maybe save your current league folder in a safe place and then unlicense, uninstall, and reinstall the game and see if that fixes it.
Although I don't think this would make much difference it is relativly easy to do and would cover one more of the bases in trying to figure out why this is happening to you.
