Holy crap, that's crazy. (Sorry, I don't know how to fix this, but something is definitely broken if this is happening to you.) I've had seasons where I see my team do this a few times during the season- and that's with total speedsters with good gap power- but one a game and sometimes multiples a game, yeah, that should not be happening.
