you may have a bad gpu?

obviously, you need to troubleshoot less problematic issues, first.

quality settings of gpu (outside game) may require a perusing, if you have not done so before. google any term you don't recognize and it's easy to make an informed decision on that setting and whether it'd be relevant to your problem.

if it's a new pc/laptop, there should be no issues. does the screen split, ever? like the sides beome the center of the screen? that can be a bad gpu. it's having a problem rendering various parts of the 3d graphic and that's not typically a great sign.

tearing is not a problem and can be fixed with locking fps to vertical refresh rate (typically 60).

if reducing quality doesn't help, try increasing it -- especially if you bought mid to upper range hardware. ootp is not too hard on 3d resources, i don't suspect. do keep FPS locked to refresh rate though. if it can't keep up, look for "opengl triple buffering" option in gpu settings.
