For what it's worth, I've had games where I've "tried" to bring in a reliever from the bullpen and it didn't work; I was just mis-clicking when I dragged the reliever from the pen to the "pitcher" spot on the screen so instead of putting him in the game I sat him down. Easy to miss sometimes.
