Turn commisioner mode on, then go into player profile -> profile editory.

more things are visible, may want to take stock while it's on and opened... you cna fix most things but not all... and those you can't fix, typically you can find a workaround (like the other post in new to game section about accidentally enabling ai-roster delegation and "fixing" prospects that are erroneously on the 40-man = hack fix, not a perfect fix one)

thousands of players... soem are a bit off, especially lesser known ones like amatuers or younger players... if it's a regional name just know no one knows them but people in your region. bound to be some mistakes in the data collection (human error, not the computer's fault)

e.g. the young tiger's pitcher had a velo bump during this release cycle.

if he hadn't won rookie of year and played well for ~2years it would have taken even longer, if it happened at all. same with jd martinez... it took 2.5 years before they bumped his ratings

(that's a compliment to ootp, btw... knee-jerk reactions to small samples are wrong, even though in the meantime i did 'fix' jd martinez each year... amazingly almost exactly as he was changed to... although he may get an even bigger bump after this year if it didn't occur already.. the guy has really phenomenol power)

most of this type of thing is automated, so certain things will occur wihtout oversight that may be wrong, but it's a small %. they do their best to improve it.
