Possible answer to #2 - Your starters might be playing too well, eating up too many innings. If you have some relievers set to be used more often, they will get the bulk of the opportunities to start with, and even a higher percentage if there are very few relief innings to come by. In one of my seasons, I did have a similar thing happening with one reliever without any of those issues (all my other relievers had between 10-15 innings), and I couldn't come up with a reason. I played through May, and he caught up a bit, getting a lot of innings in a short period of time.
