Top 9th, score tied, 2 out. Home team pitcher is injured on the final pitch of the inning getting the 3rd batter out. Game forces pitcher to be replaced. Home team scores in the bottom of the 9th. Replacement pitcher gets the win without throwing a pitch.

The replacement pitcher should only be credited with the win if he actually pitches. Indeed, in a DH game, where the replacement pitcher doesn't have to be inserted in the lineup, he shouldn't even appear in the box score in this situation.
