You need to find or make an image file in .png format, about 40 by 40 pixels.

Rename the file award_custom.png

Close the game.

Go to the skin folder of your choice and copy the image file over the default in the images folder.

Delete the skin.bin file and restart the game. The game rebuilds the skin.bin file using the custom images.

My man Chris Anderson has won my custom award, the Most Popular Player (heart), four times!
