Really going to depend on your roster, but if my starter's status is 'tired', with at least 2 outs in the 8th, I'll bring in my closer (unless he's completely cruising). Anything less than 2 outs, I bring in my setup man. But generally, go by your starter's reputation and stats. If he's a mediocre rookie or something where he's simply having a good game, pull him before he ruins it for himself. If it's a Kershaw-MadBum style pitcher, perhaps leave him in. A good indication to see if your pitcher needs to be pulled are the walks. If they start walking guys in the 8th, bye-bye.
