Advice for sending in your closer/relief pitcher

Anyone have any pro tips on when to pull your starting pitcher? I'm finding when I pull a tired SP and my team is in the lead, my closers or rielievers give up a few runs almost immediately. My favorite moment is being up by 3 runs, bringing in reliever who allows bases loaded, then I put in a closer which led to a grand slam.

I've been sticking to the replacements save percentage, health level and low ERA but it's not working out for me. Maybe push a tired pitcher a little longer? Thanks all.
