For one thing they weren't throwing as hard as they could on every pitch. They knew that 89-91 was fast enough if their secondary pitches were good. Also, they trained to go the distance. When they took the mound it was with the expectation that they'd pitch well enough to finish what they started so they conditioned themselves accordingly. Pitchers in that era also used full windups and pushed off with their legs far more than they do today. As Steve Stone put it, "If your legs can carry you for 8 innings, your arm can carry you the ninth". Many pitchers today simply turn sideways and gun the ball in as hard as they can without winding up and driving with their lower body. Despite all of the advances in medicine, every year many pitchers are lost to injuries, so pitch counts and/or limiting innings isn't the answer.
