I think I've found the issues -- one is an obvious bug, and the other is likely a design issue.

Bug: After a league is created, player pictures are created on demand, even if you have them set to "Upon Player Creation" in the Facegen settings.

Design: When a coach needs a facegen, the game looks to see if there is a player facegen, and will create a new face if there isn't an existing player facegen. However, if the game needs to create a player facegen, it doesn't look to see if there's a coach facegen already for that player.

The issue with coaches having different faces appears to stem from their player facegens being created after their coach facegens. Fixing the bug above would resolve the issue for people who have "Upon Player Creation" enabled in the Facegen settings. For those who have "On Demand" enabled instead, the design issue would also need to be addressed. I'd be content with the bug being fixed for now, if the design issue is too much work. Alternatively, forcing an update/generation of player pictures once a year might be a workaround for players who want their coaches to have the same faces going forward.

In my test league, the coaches who had the same facegens as their playing days were created when the league was created (they were in the inaugural draft or with feeder teams), or had a baseball card generated at some point during the simulation. I've attached screenshots of a couple of these players. The game seems to create faces for everyone just fine when you start a new league, but it doesn't do that automatically afterwards like it should.

This also explains what I've seen in my fictional league -- coaches who played in the majors for a while or were in my organization at some point had the same faces as their playing days. Coaches who had brief playing careers were more likely to wind up with new faces. I also seem to remember forcing updates of player pictures periodically in my initial years with the league, which might explain why I've only seen this issue recently.

Hopefully Markus sees this and can at least address the "Upon Player Creation" bug mentioned above.
