Jim Unknown Issue ?
Sounds like you have the dreaded "Jim Unknown" syndrome. I have that issue in my league also when I run a sim and seems to create Jim Unknowns in the minors for minor league teams missing enough players to complete lineups. Does not matter if "ghost players" option is used or disabled.
