The option on the strategy page reads "Leading by X runs or less". Thats how its worked in past versions and how I would expect it to work, though it would be nice to have an option that specifies "any lead" if that's how you make your defensive substitutions.
No, its not a huge issue, just something that would make the game better if it worked.
