you can set the region to the state level (so you only get Ohio cities, for example). Other than that, it would be a manual process to enter alternate cities. Not sure how the market size works for cities that are manually input that don't actually exist in the xml file (might be able to make your own manual adjustments on that as well... not in a place to test it myself right now).

If you feel like poking around the world.xml file, you can always go in there to see which cities are already input into the game (with population sizes/etc); and if you're feeling ambitious, you can add in more small towns if you want to put them in with their actual populations/etc.
