This is all anecdotal, and I have no way to tell if this is "realistic" or not, but it was enough to get me to switch back to the Normal injury setting. I've pushed in the past for two separate injury options - Injury Frequency and Injury Severity - that would allow you to play with a lot of injuries, but have most of them be short-term ones.

Tried high. Had all five starters down at one point. Not doing that ever again.

According to OOTP itself, OOTP MLB play (modern and historical) outnumbers OOTP fictional play three to one.
