One cool customization that would really spotlight OOTP's customization advantage over it's competitors would be the ability to modify baseball rules.

Just like baseball did during it's growth.

A walk could equal 5 balls, instead of 4.
A strikeout would consist of 4 strikes... or none at all, have to put ball in play.
A ground rule double = a hr.
A hit batter equals an at bat.
Balks don't exist.
etc, etc...

I think that would really up the ante in fictional lgs and historical replays (even more so if the pre-1900 engine gets redefined).

Just my humble opinion... I think someone has mentioned this before too.
