The AI considers any player at less than 100% fatigue "tired", and therefore replaces him with the "If Starter Tired" option on the depth chart, unless that option is already playing another position in that game. I roll with that, since position players tend to stay at 100% long enough to get plenty of games played that way, and I've seen players with fatigue under 50% or so get injured far too often. It also gets the subs some playing time. Replacing the starter at 99% works well for me, I'd let the AI handle it, then set the lineup to your liking with the substitute player in for that game.
