Regarding the four pitch outing, that happens occasionally for no apparent reason when you're clicking "Finish Today" and then looking at the box score. My guess is that if you saw the play by play you would see something like "uh-oh, Smith is holding his elbow, he may be hurt, they're taking him out of the game" but then the next day it turns out to be nothing.
