First you need to open up the preferences screen. Click the facegen tab and set the last option (Picture Search Mode) to search for photo first.
Next you need to go to your saved game's news\html\person_pictures folder and delete those. Close and restart ootp and all your real photos will be recreated as long as they're named correctly. You shouldn't have any more facegens showing up.
BTW: Away uniforms do not affect real pics..only facegen.
