Starting with an expansion team is the only way I play OOTP now. I start it right out of the 2013 Quickstart menu. If you go in to your league options you will see the option there. You will have to expand by at least two teams. The game will guide you through the parameters. The draft process is quite well done but does not entirely simulate MLB expansion.

Once your team is created you may want to adjust your staff right off the bat (scout, coaches etc).

Also, check the schedule in the pre-season each year. The 'defaul' expansion can leave you with a lot of two game series. If you generate a new schedule (done for you with one click) you will have a traditional style schedule.
