Simple setup starting in 1976 using real players with fake teams. After the league has been setup and a season has been simmed, the league reverts back to real teams for the next season?!?! How do I get around this issue and what on earth is causing this?

You have the box selected for expansion. When that box is selected OOTP is going to expand your league according to real life expansion. It will also relocate teams(Brooklyn to LA, New York to San Fran etc), change team names etc. If you don't want this to happen, uncheck the box for expansion. You will have to handle expansion yourself, but your fictional team names will be safe.
