I take it series-by-series. I "play out" the first game of the series (but usually I only control substitutions and leave strategy to my manager) and sim the rest.

I don't know what you mean by this exactly. But I tend to look at things team-by-team, or player-by-player. For example at the beginning of the season I like to write up scouting reports on each team in my sub-league/division. At the all-star break I'll look for narratives -- what players are over-performing, or under-performing?

I evaluate all of my minor league players in the preseason, and decide if I want to promote, keep, or cut (generally I don't demote, I just cut). Usually this is just a quick evaluation based on their prior year stats. If it's a "prospect" with actual major league potential based on his scouted ratings I'll take a closer look to see where the minor league report recommends placing him and if I agree based on the ratings and stats. I check again in June/July to see if anyone is really dominating their level and merit an in-season promotion (often these are players who had fared poorly at their level last year and are repeating).
