Unfortunately due to some technical coding reasons rehab assignments don't quite work like they do in real life. In the real world, the player is still on the DL when sent on a rehab assignment. When the rehab stint is over, and it seems he's properly recovered, then he's activated from the DL. In OOTP, the player comes off the DL and is then sent on the rehab assignment.
Note also that in real life the DLs stop operating after the end of the regular season, and don't start up again until early in spring training.
