If you use the instant play-by-play speed option, you can manage games very quickly. You have to be careful, though, because you can get into a groove and accidentally hit a key too quickly and forget to pinch hit for someone or use a different strategy.
If you're using real life transactions, your teams will perform largely as they did in real life. Yes, there will be variation, and mathematically that can be much more significant than people realize. But simply changing lineups and using some different strategies probably won't give you 10 or 15 more wins than you would have had otherwise. If it was that easy, real life MLB teams wouldn't be as bad as some of them are.
