It probably is just taking too long for the server's liking. It updates the player pages as it goes, so it's likely that it gets to most of the players pages and so it "seems" like its working, although a few players might be missing their links.

If you can up the timeout on the server, that should fix it. Otherwise, be content that it does most of them. Note that the "retired players" option is only for the report I think, not the links. I guess I could hook it up to work for both (it was needed to conserve memory, not time. Memory is not an issue for generating links).

The icon images for the All-Star Voting (positional and instructions) are not showing up. Is this part of the same issue?

No. I know for some of the older patches I forgot to bundle the image file into the OOTPOU.zip when I built it, but it is there in the latest patch. See if you have the file OOTPOU/allstartImages.zip. If you do, it should auto-copy and extract it to the <webserver>/OOTPOU directory when you generate the ballot. If not, manually copy it to that location and extract.
