Each year before the Rule V draft I look to make sure all my players that I want protected are protected. I then glance through the rest of the league to see who might be available. Every season I see a few players who are on a team's Active Roster but not on their 40-man roster, which should not be possible.

See MR Lien-ying Sang in the image below. On the Oakton active roster but not on their 40-man roster and shows with the # after his name indicating he needs to be protected on the 40-man roster.
