I haven't had this problem... in fact I get the results you describe as being desireable... (This through about 10 seasons worth of playing out games with OOTP3 and OOTP4)

If anything I find my pitchers either walking the player or retiring the hitter as he takes a hack at something bad.. so I often use this strategy against hitters on a hot streak or who tend to just kill me. (Unless I can not afford the walk (i.e. bases loaded))

So it looks like you just hit a bad run of luck.. or your pitcher was tired or not pitching very well to begin with...
