Earned run bug:

Runner on second, no outs. Batter singles to right. Runner goes home, throw goes to home. Right fielder airmails the throw. Runner scores, batter ends up on second.

In real life, this is always - 100% of the time - an earned run, and in most cases it wouldn't be an error, since there's nothing unusual about a batter advancing to second on a throw home. (There is some discretion there for the official scorer, though, because sometimes it's clear that the batter would not have advanced but for the bad throw.) Because throwing a runner out at home is a difficult do-or-die play, they don't give errors for failing to get the runner at home - only for allowing other runners to advance.

The game scored this an unearned run because of the error.
