One out, runner on third. Ground ball to the middle infield, runner heading home, throw is to first, throw goes wild.
In reality, while there is some discretion here for the official scorer, no scorer would give an unearned run when the throw wasn't even trying to get the runner at home. The scorer's job is to try to recreate what would have happened without the error and designate the run accordingly. Here the run would certainly have scored either way, so it should be an earned run.
OOTP scores this as an unearned run, as it does in all cases when a runner was on base or at bat when an error or passed ball occurs.
