this happens a lot more than is should, i was watching watching an mlb blooper dvd that was made in the mid 2000's and they showed at least 5 where they tossed the ball in the stands after the second out. on one the guy handed the ball to a kid, took 2 steps back, then realized what was going on so he went back and took the ball back.
I personally would like that to randomly happen like in 1 our of a few thousand games in pbp mode in ootp
