1 Face a batter, or
2 Record an out (can be done w/o a BPF)
It's possible to get a blown save without doing one of those things (run scoring due to SB, WP, or PB), but not a loss. Wins, saves, and holds require that the pitcher record an out.
The simplest fix would be to lock out pitching changes while batting (even batting another pitcher for a pitcher is considered pinch-hitting after all)
