That sounds like a team either has the wrong abreviation (team abreviations are used in the record book) or the more likely answer is you moved a team from one league to another at some point in the career. If it is the second case, I am not sure that is completely fixable for past seasons but should work properly for future seasons. I have a lot of experience in expansion and switching teams. Expansion I have down now thanks to Scotts FAQ, switching teams from league to league is just going to cause problems though
. There may be help on the horizon if a detailed editor is available for OOTP4. If you can't fix this issue now you
may want to save the league until an editor comes out to see if it can help you.

If you have the AI controlling your minor leagues (from the team setup screen) or you are leaving the minor league lineups blank then this is normal, but if you set minors control to "human" and input all 3 minor league lineups manually then I doubt that is supposed to happen, I have not seen it yet at least.
