I play out all my games in one pitch mode and only do the substitutions for my team. This way I don't take advantage of the AI teams. They have walked a run in but very rarely.
If you only handle the substitutions then make sure that on the 'Options & Preview" screen just before you start your game that in the 'Pitcher Warmup Rule' you have 'No warmup required'. If it's on without you using the bullpen then your pitchers will give a ton of walks. This setting should stick for every game. Just make sure to double check with a new season. I didn't noticed one season and it was 'on' and sure enough my pitchers were walking a whole bunch.
If that's not your problem then I don't know.
