I have the same issue. Imported my 40-league universe from OOTP11, and my Canadian cities are messed up. The Continental League has both US and Canadian cities, and Calgary, Whitehorse, and Winnipeg all have wacky state/country affiliations (middle eastern, asian places). All the US cities are fine.
I'm at work, but when I get home I'll check to see if this is happening in any of the other leagues in my universe.
Possible things to look at: The CBL is a inter-country league, maybe this is part of the root cause. Also, I had modified the old cities and countries files in prior version(s) of OOTP to add some smaller places.
