I've played hundreds of seasons in OOTP, sometimes my teams fell apart at the end, sometimes they got hot and came from the middle of the pack to win. I've had teams I thought were surefire World Series contenders who finished with a .500 record and teams I didn't think would even break .500 who had deep postseason runs.
Just like real life baseball, things don't always work out the way you expect/hope they will.

Yep, it happens. I played something like 5 seasons in a row where my All-Stars got injured the week before the playoffs began. The first two times weren't a big deal. It was seasons 3-5 that freaked me out. Then, in year 6, no injuries and a WS win.
