Has it happened 3 times with the same league and at the same spot? If yes then that league may need to be restarted, but if this has happened at different times and/or in differnt leagues then I'd make sure you had plenty of free hard drive space (my own limit is a gig free per drive) and then run a scandisk or Norton disk doc program.
It is wise to keep league backups every once in awhile. In OOTP2 I had many crash issues and saved a copy of the league each year. I got lazy in OOTP3 and saved backups less frequently because of no crash issues. If you are within minimum system specs and make sure there is always plenty of hard drive space empty, make sure you have no operating system or hard disk problems, and ideally don't run heavy resource intense apps (like Winamp etc.) while playing OOTP4 then I think there is a high probability of zero game crashes.
