Just a few ideas from my own personal experiences:

1. State League. Pick your favorite state and section it off into divisions keeping travel distances in mind. Another spin would be to create 1 league with 2 divisions of up to 10 teams in each. The only possible drawback to this, depending on how you look at it, is that there are no playoffs. The best team in each division plays in the championship game, which makes the entire season a playoff of sorts.

2. National League. Start with 2 leagues that have either 2 divisions of up to 6 teams each or 3 divisions of up to 4 teams each. Pick one city from each state that will be represented. At a good point in the career it is a good idea to expand to include 1 new division in each league so that one league gets teams south of the U.S. boarder while the other gets teams from Canada.

3. World League. Create 2 leagues with 3 divisions of 6 teams each for a total of 36 teams, which will be represented by 1 country each. Set the minimum player salary to $100,000. The idea is to create a level playing field for all the teams and to hopefully come up with something that could be considered an Olympic type league. Use 1 ballpark with average ballpark factors so that there is no advantage to any particular type of player or team. Make it a Dome or a Retractable Roof stadium and make the average temp 75% with little chance of rain. Use the flags from each country as their logos.

Note: It is sometimes a good idea to make sure that all divisions have the same number of teams. This can result in a more balanced playing field in regards to the postseason.

Note: Try to limit your leagues to no more than 36 teams total. The game will allow for up to 40 but it has been determined that this is not a good idea since there are inherent problems that need to be fixed first. For example, any matchups over 36 teams will not be displayed on the schedule screen and dependant on the structure of the league there may be teams that scroll past the bottom of the standings screen.

Hope that helps a little. I would be happy to add more insight as needed.
