I understand a lot of these are rare occurrences in real life baseball but they do happen. I've played 100 games so far in pbp mode and these just dont come up.

I've definitely seen a couple ground rule doubles, runners thrown out stretching doubles to triples, ITP HRs (one in very first game of XI), and Strikeout/passed ball batter safe at 1st (once only). Can't swear I've seen runners thrown out stretching for 2nd, or successful steal on pitchout (though I think I remember one or two, won't swear to it).
Difference is, I play full pitch-by-pitch mode, not one-pitch. Don't think there's any difference in the engine, so should be same between the two modes. They (most) happen, but are suitably rare.
