Since OOTP 11 was released I have seen many posts and confusion about FaceGen, Fictional pictures, and getting it all to work correctly. Apparently there is a bug that is affecting all of this which SteveP said would be corrected in a patch.
I think that first of all we should define what we are talking about

In the past few years of OOTP the FaceGen program was used to create "fake" player pictures.
Additionally, this year FaceGen pictures have been created by many people over at the SoxxCountry website using a program called FaceGen Modeller, which creates FaceGen pics that look like "real life" players based on their real photos. These have been made available in two mods (H.F.P. Historical Facegens, and H.F.P. 2010 MLB All-In-One) which can be downloaded within the game using OOTP > Add Ons Central. Make sure you have Padresfan's server listed in order to download the mods.

So lets get the names straight:
Fake player pictures for fake players, generated within OOTP = let's call Fictional Pictures.
Player pictures that look like real players, generated by people using the FaceGen Modeller Program = let's call FaceGen pictures.
I was able to get both H.F.P. mods to work by the following:
Install OOTP 11
Import a Fictional OOTP 10 League into OOTP 11.
Start the Fictional League in OOTP 11
Within the Game go to OOTP > Add Ons Central > click on reload server list
Download both H.F.P. mods
Install both mods
Restart the game
Setup a new MLB 2010 season - restart the game - you should be all set with FaceGen pics.
Setup a new quick start All Time Greats season - restart the game - you should be all set with FaceGen pics.
Just make sure that OOTP Preferences > Facegen > Facegen enabled is chosen. Don't worry about the settings in Game Setup > Player & Picture Options > Fictional Picture Options. These settings only affect Fictional "Fake" players.
I think the bug comes into play so to speak, when setting up a Historical League. For whatever reason, it pulls in Photos instead of the FaceGen pics.
Anyway, this is my understanding of the problem which I figured out through trial and error. Please correct me if I am wrong.
