The first license isn't lost. It's just that the PISD system thinks it is still in use. You should go to their web site and ask to have it released. This is worth doing, and shouldn't be a problem.

The only remaining issues are: What is that icon on my desktop? Is it the game (in which case I should move it), or is it an alias (in which case I can trash it)?
I am 99% certain that it is an alias. The installer does this in default mode. You can check by clicking on it and then clicking on Get Info.

And what do I do with the folder in Documents? Do I need to drag any of it to Games? Delete it? Just pretend it doesn't exist?
If we are talking about the Folder I described before -- the one that is titled Out of the Park Developments, with another folder inside that is titled OOTP Baseball 10 -- do not trash it! It contains at least one hidden file.

Anyway, I'm now very satisfied with my OOTP Developments experience — pretty good from a guy who doesn't work for the company. Still not happy with PISD, but that's not under your purview.
IMHO, don't be too quick to assign blame in one direction. I will admit to having some significant frustrations with the licensing system. However, in my experience, the guys have been pretty diligent about fixing problems that were systemic in nature -- that is, which affected lots of Mac users. It's also clear to me that PISD isn't making all the decisions on how their security system is used or incorporated into the game. So, it makes sense to cut the guys some slack. I have.
