Here's my take on it. Someone else may have a more authoritative answer. I think fan loyalty is simply a characteristic of a given city, like market size. It doesn't change. In both cases -- market size and fan loyalty -- there is no guarantee that these will resemble what you see in real life, unless and until they been modified after game creation by someone acting as commissioner. You have to be somewhat careful about it, however, if you do make modifications to those (the AI may do strange things if the financial drivers are changed a lot in one season).

Fan interest obviously does go up and down, as a result of a team's performance, what happens with popular players, etc. I think fan loyalty influences how much fan interest can change as a result of events. For example, if fan loyalty is high, fan interest won't drop that much if the team is doing badly.
The combination of market size and fan interest are what produces the ultimate revenue for a team each year, through media contract size, ticket sales and merchandise sales.

Season tickets are not a factor in OOTP. Don't know anything about the other, but it seems safe to say that the answer is no.
