Yes, you'll always need to set up OOTP to return 2^n teams to the playoffs, and then work out how many series to delete.

So if you want 3 rounds per sub-league before the world series you would need to set up a custom playoff in OOTP to allow 8 teams per league and then depending on how many teams you actually want to qualify you'd need to delete

5 teams to qualify - 3 series per sub-league need to be deleted
6 teams to qualify - 2 series per sub-league need to be deleted
7 teams to qualify - 1 series per sub-league need to be deleted

At the moment don't delete all the series inside a round as there isn't a way to add one. If you follow the instructions in the above post about doing an ML one you'll get the rough idea how to adapt it to any structure.

With regards to teams thinking they've made the playoffs, I'm sure there is some change to AI close to the end of the season, but I don't think there's any other issue to them (it's not recorded in history as having made playoffs unless they've actually played a playoff game)
