This is why I ask, (and I play with 'instant' pbp, so the game goes by fast!) If there ARE errors when this happens, well in the limited play I have seen with OOTPX, I havent seen errors go up compared to other versions of the game. I think this is just a dialog string reiterating too often.
HOWEVER, if there is no error when this play happens, well it DOES happen alot, but likely in older versions of the game would be simply a hit.
I think this string was added to try to make things more exciting...

There is no error on the play since it's extraordinary effort.
In earlier versions it would just be a hit. They're in fielder effort missed, so in OOTP9 you would have just seen: " but he can't make the catch..." or one of the other 15 or so in there.
It was added to offset the extraordinary catches, where he dives and catches and rolls up and all is fine. Now when you see him dive and catch, he could lose it when he hits the ground or the wall.
The frequencies have been turned down for the next patch.
