I have an above average (60 out of 100) on first, a pitcher who is below average (48 out of 100) holding the runner and a catcher with a 1out 100 throwing arm. I don't see why he doesn't go when told.

That's not quite how baseball works. Managers don't fine players for not going on a pitch. Generally speaking, the steal is up to player and not the manager. I think if he
never went when you put the steal on, you might have a problem.
